Creating a new customer form

I would like to add to cutomer forms to my site i have already added the links from the information box etc and have the pages ready i just dont know what to add to the new pages to make a php form that gets emailed to the store owner. I need a feedback form and a customer wants list where customers can request products for the shop to sell.

 

I though if i copyed the contact page form and added in fields and made sure they were included everywhere they should be that it would work but unfornuatly not.

There is already a Customer Feedback Form contribution. If it doesn't do exactly what you need, then it might at least give you what you need to complete your forms.
