Customer order had incorrect Ship To address and incorrect item

Once in a while, the following happens in our OSCommerce 2.2 (CRELoaded) store:

We get the order from an online customer, and the ship to address is not the customer's billing address. The alarming thing is, the customer doesn't know this individual and did not input that information; the item is also not what the customer purchased. We find no record of the Ship To person; my thought was perhaps the last online order didn't clear out of the /tmp somehow (?) and their info and item got melded with the new order. However, the individual does not appear in either customers or orders.


I am unable to recreate the situation but it has happened 2x that we know of.


Ideas? Wrong settings re: caching or something else, on our part?
