Can't Put My Finger On It

I'm totally losing the attributes after the customer clicks add to cart. Where should I be looking for this error. I have highly modified my store and I can't see to find it... i just need some direction.

what changes you've made to the attributes fields? The identifier array is not populated so that's what is causing a problem there

 

from your html code I could see

<select name="id[]">

 

It should been something like <select name="id[4]"> for example
