Major Problem Mail/feedback not getting sent to me.

I have two pages in my site where people can contact us and also request a catalog to be mailed to them. A few days ago I stopped getting the requests which are automatically emailed to me-one at a time.. I got in touch with the host in order to find out if there is a problem on thier end.

 

Customers are able to ask for a catalog online. it is captured in the customer admin section but the requests do not get emailed to me as before.

 

I appreciate any ideas or suggestions.
