how do i add a required check box to a page?

hi,

 

how do i make a check box on the Payment Information page required and so it sends the information that it has been checked. i placed one there but i cannot figure out how to make it required and to send the field in the merchant email receipt.

 

i searched around and came up empty.

 

this check box was suggested by our payment authorizer so that customers are aware and to protect ourselves.
