Selecting Multiple Items

I have a need to allow customer to select multiple varieties of the same product in the product info page.

 

What I have is jewelley pieces that come in a variety of colurs, as well as different sizes. What I would like to have displayed in the product info page is a list of colours (not a pull down) with multiple select, a list of sizes in the same manner, and input field for quantity each.

 

Once the buyer selected colours, sizes and enterd quantity for each, and clicked the add to cart button, each variation will be enterd as an item to the cart.

 

For example, if the buyer chose colours red and blue, sizes M and L, quantity 2 each, there will be 4 entries in the shopping cart: red-M qty 2, red-L qty 2, blue-M qty2, and blue-L qty2.

 

Does anyone know if there is a contribution out there that satisfies these requirements? Thanks. :o
