Add extra questions

Hi, hope 'nana' can provide some advise regarding about this contribution. Just install this contribution smoothly and i amsure i follow the instrutions correctly. In admin/questions, there is an error "1054 - Unknown column 'sources_id' in 'field list'

 

select sources_id, sources_name from questions where question_number= 2 order by sources_name limit -10, 10
