Contributions (Excel Import 1.51)

I installed the Excel Import 1.51 contribution with no noticible problems. However, I cannot seem to properly utilize the excel form that came with it. Each time I try and upload a sample product, it does not populate my product attributes correctly. I am at my witts end. Please help. Any info would be appreciated, even a form would be nice to see from someone else who uses this mod.
