Product Options Text Box

Hi, I am looking to create a text box for my cart where someone can enter a personalized option. I am selling products that can be personalized and I want to be able to have them type in their own text as one of the options. Can someone help me with this?

 

I am not looking to have this info in the customer fields, I want this option in the product selection process if possible.
