Product Attributes - Need to be able to type in

I searched this topic and learned enough to know that it can be done, but I can't figure out how to add a field where the customer can type in information about an order. I have drop down boxes for things like size and color, but I need a field where the customer can type in their child's name for cutomization. Anybody know where I might find detailed instructions?

You may be able to use something like this, or else you can just add some text to the 'Comments' box during the checkout procedure to get people to add their message there.
