Product Attribute Text Box

I have a website named www.ledframes.com, i have the products of led buckles. For each product i have to insert the product attributes as Belt Color , Belt Size and Name of the Person in the Led Buckle.

 

I have created Belt Color and Belt Size for all the Led Buckles, but without name i'm unable to service the Customer.

 

So how to insert a new Product Attritube for my products, and especially i need to insert a Text Filed and Text Box, for example

 

Name: PEPSI

 

so the text field should be displayed as Name and the Text Box should be maximum of 7 letters which the customer will give.

 

How to do this, Please help me any one can have the solution for this.
