Problem with Canadian shipping....

We have our first US customer signing up to purchase one of our subscription products - the problem is she can't seem to get past the Delivery Confirmation part of the checkout process.


We are using the Canadian Province Shipping v0.9 contribution - and have it set up with 3 Canadian zones and one US zone.


I have added a US address to my test email, and I get the same problem - it gives me a total, and then gets to the shipping screen, and when you click Continue, it brings you right back to the shipping info (delivery confirmation) screen.


With this contribution, the first question is "Enable US Regions Method" and it is set to "1".........I have tried changing the amounts in the zones, adding more amounts to it...but nothing has worked.
