Let Customers Specify Product Options??

hi,

 

is there a way to let customers input the product options??

 

I was thinking of putting a text area where they can give special instructions for each product, but how do I do that?

 

any one has the same issue? solution?

I tried to installed but it does not work on my admin.

 

I just downloaded the file, replaced it with the one I had, tried to add a field, but it gives me an error.
