Quantity for Product Attributes Mod Help

Hello, I'm a super newbie to this stuff and have done pretty well figuring out parse errors, learned to compare files, etc but now I am stumped. I installed Quanity for Product Attributes all the way to the 2nd fix on 7/28/05 and I am getting the following message on every product (plus at the top of the screen) when I go through the checkout process--when I get to the screen right after payment type the errors pop up.
