Ip Showing instead of domain name

[OSVS]

I am having a problem with my ip showing up instead of my domain name after I click some links:

 

Example - 64.257.1.1/catalog/checkout_payment.php

 

instead of www.mydomainname.com/catalog/checkout_payment.php

 

Also, where do I change the "Title" for the checkout process. Presently all my titles are the same, until I go into the shopping cart and procede to checkout.
