We have some products that we requier the customer to give us name, age,city etc. for what they need printed in order to personalize their products that they buy.

This one is great if you need the customer to upload an image File Upload (Option Type Feature). It adds the basic features of the first contrubution and adds a upload attrubute. I have installed both, they are not easy to install but if you have a business that personalize products they are worth it. Good luck, Raymond
