Meta Tags for each page

In the index.php I can set Meta Tags that will be the same for each and every page. Is it possible to 'split' this functionality and generate unique Meta Tags (specifically Title, Keyword and Description) for pages within the Catalogue.

 

The aim of this is to improve Search Engine ranking as a targeted page title, keywords and description that match the page content will rank better. On the home page, the (in particular) page title and description are more generalised and less product specific.

I used Header Tags Controller you can set all pages the same or do them individually.
