shipping option display

hi,

 

i've got an online store up and running at the moment, and i've installed the australia post shipping options, and turned them all on in oscommerce admin section, but there's no screen that appears during the checkout process where the shipping option can be selected.

 

also, i've added in the insurance option, but it doesn't add the cost of shipping to the price of the invoice. i'm pretty sure i followed the instructions properly.

 

i'd really appreciate it if anyone could take the time to have a quick look for me - even more so if they could outline my stupidity for me.
