Create account box configuration

I want to remove the newsletter box in create account page.

I have used the http://www.oscommerce.com/community/contributions,1649 but that did not do it.

 

I also want to remove the fax number box and to move the phone number above the email addres in Personnal detail.

 

How can I do that, I have searched every where, I cannot find it !
