installing PAYMENT via fax/phone problem

hi,

 

i have search forum to find answers but non available thus it seem everyone else are ok when installing this function BUT ME...URRRR!

 

anyway, with this payment method when customer checking out they are able to chose whether to pay via fax/phone for credit card details or others --> at check out customer can click on button that said 'produce form'

 

the problem is when click on my ''produce form' --> open up a new screen but form does not appear!

 

on new page browser show www.yourdomain.com/FILENAME_CCBYFAX_FORM

 

the browser should show www.yourdomain.com/ccbyfax_form.php !!!!!!!!

 

 

please point me to the right direction anyone please....
