Try restoring your root level index.php file from an untouched copy. It does appear as though an extra table cell has crept in somewhere, or else a colspan define has been removed. I haven't made any changes to this file but noticed that there was no definition for: define('TABLE_HEADING_IMAGE', ' '); Which is my first colum selected.
