How about creating a NEW item with the following description with a price that is 1.5 times a single item price?

 

Ok, so its a workaround and it won't 'work' if people buy two single items, but it is one way to do it :)

Good point. I may create such an item and limite one item per customer.

Have you looked in the contributions? I recall seeing a couple of two for one price ones. I have not used them so I am not sure if any of them would do what you want but it is probably worth looking at.
