How do I get a mailing list from my database?

I'd like to send a "thank you" postcard to all our customers. How can I generate a name/address list from the OSC database?

 

The address_book table seems to be the closest thing but it's not showing any STATE info, which is odd since I do have it through the admin interface.

 

Can anyone tell me a tool that might do this nice.y so I don't need to extract the info from multiple tables?
