"How did you hear about us" contribution

Hello everyone, I really need help with the contribution "How did you hear about us" found here:

First of all, just to let all of you know, I am using STS. I installed the how did you hear abuot us contribution it by going into each of the files that came with the contribution and looking for the edited tags. Then I put them into my current files. The contribution seems to work fine, its just that my site shows this at the top:

Not sure but take a look in your sts index and see if you've duped up some of the $headcontent. Otherwise post this (and search) the STS support thread. Lots of knowledgable folks in there.
