Changing the address layout

Hello, I'm sure this has been covered but I'm wondering how to change the address layout when you create a new account.

 

I've searched but everything is pretty old and referances an account_details.php file which I don't have.

 

I'm running ms2.2 the site is half up at meandmymachine.com

 

If someone could point me in the right direction that would be great.

Oh, one more thing, how would you put United states at the top of the the country drop down box. Again everything seems a little outdated.
