How to alter contact us form include attachments?

I want to make a "Link to Us" form page and am using contact_us.php as a template. I think I can muddle through adding the extra text input fields on my own, but am unsure of how to ad a file attachment feature to allow responders to attach their desired banner image and submit it with the form.

 

I have seen at least one OSC site that has such a form feature, but despite giving it the "college try" for 90 minutes on the forums, I haven't found an answer for my situation.

 

I have a solid amount of experience with adding OSC contributions, but am not a php programmer, so I'm looking for as user-friendly a solution as possible.
