Form Help Please!

I am trying to make a form that would bring up the account_history_info.php file with a specific order number. (example: account_history_info.php?order_id=12345)

 

I am trying to be able to have customers insert into a form field their order ID number, click on submit (or whatever) and be taken directly to the specific order.

 

I am not very familiar with php, so please be as detailed as possible with any help offered.

 

Any ideas... Any help... would be much appreciated.
