Payment module for the Cambist payment gateway with CVV2 and AVS.

Cambist Payment Module v1.47 6 Oct 2005

Added two parameters that can be changed via admin when installing the module. They are to set VoidIfAddrInvalid on and off; and to set the email address Cambist sends approval confirmations to. Also, it now gets the store name from the database to send to Cambist so it doesn't have to be changed in the code.
