Survey Product Generator - Sent by Email!
This contribution prepares an editable 5X5 question & answer survey to be sent by e-mail to the customers that complete orders. The system generates an e-mail with a link that is automatically sent to the customers when it arrives the timelapse choosed in the admin.
You can put the system ON&OFF at any time. Just to make it simpler, the check for the date is done when any customer log into the page. A code inside checks for the table and make the sendings if everything is right.
When the user clicks into the link, it goes to a page of the Web where answers are displayed. All info is stored in DB.
This system is useful just to make a simple survey to all customers that is sent with no human being. You can check all results and state of the system from the administrator.

Added a code in checkout_success.php, depending on the way of payment, there were duplicated entries. Now a comprobation query is done, it sholud not happen again!.
Updated bug:
Changed the content in checkout_success.php, because sometimes the query was duplicated.
