Login Box w/My Account
The version is set in keeping with the code I am working on for other Add-Ons.
This Login Box is both a Login Box and a My Account Box.
It does not show on Login or Create pages. When Logged in ... it is a My Account Box. When Logged off ... it is a Login Box.

I took the previous (non polish) version of this contribution, and added a few things:
Under the sign in button, I added a link to the "create account" page, for new customers. Makes the sign up process more visible, and on every page.
Also, I added a link to the password retreaval page, in the event that an existing customer forgot his password.
This is my first contribution, and I'd appreciate any criticism regarding the PHP standards compatibility of my work.
