Individual Product Order Total Fee
This contribution borrows HEAVILY from Individual Product Shipping Prices v1.0. I operate a rental site, and I needed a way to calculate the shipping insurance based on the value of each item, and not the order total (rental fee). This allows you to set a fee for each product that is added into the order total. I calculate the insurance cost for each product and assign it to each item. You can use this for insurance, special handling fees, etc.
I'm no programmer, just a guy trying to set up an online shop, so the code probably isn't perfect.
Credit goes to the original writer (D. M. Gremlin) and those who graciously helped me on the support forums!

I've modified version 2 in two ways...
* Revised the code to use the more generic variable "extra_charge" instead of the original.
* Not display the extra charge if the value is 0.
