Added a drop down box for Reason of contacting us... to the contact_us form...

New Email Subject Line Reflecting Enquiry Reason. Just a simple modification to the original author.
Complete package with more detail readme file.
No SQL is needed, very simple modification that can be done in 5 mins.
(I use single left column in my store)
