Using The Mod:

Min or Max Operation: (MAX*) Amount=500 -> only show the cc mod is the cart total is under $500
Min or Max Operation: (Min) Amount=500 -> only show the cc mod is the cart total is over $500
Set Defaults:
Min or Max Operation = Max
Amount = 500
