Catalogue Request V1.0
This is a Catalogue Request Infobox and a Catalogue Request Form.
It would look better If I added the page_background image of the catalogue that apears at the top of the form to the infobox but I was having trouble geting an image to apear and link to the form correctly as I'm a novice at this thing. So please email me if you have a contribution to make to it.

Here's my take on the catalog request. I think it's a little more robust because it has an admin back end to process and export catalog requests as a clear text file.
A slight update on the contribution.
Needs a little work on the Form. Any help would be appreciated.
