The order total module included inside this contribution is very useful and works for all payment modules, not only the "contremb", so
I made a separate contribution of it. I also updated it to really work with tax class and Credit Class.

This is only the contremb payment module with one more admin parameter to set for which shipping method this module will be enabled.

Fix a error in the install file.
Now the payment charges are correctly displayed and added on the order total.

This contribution is only a package with a duplicate cod.php called contremb.php and a ot_fixed_payement_chg_update pre-configure for the "contremb" module and updated.
