v1.2 adds the ability to produce a printable fax form from the account_history_info page for
orders where ccbyfax was the chosen payment method.

Note: See the readme.txt or the forum for a full explanation on why my claim "where ccbyfax was the chosen payment method" is not totally bomb proof.

I have also made some minor changes to the other files:

- Some minor cosmetics on the fax form.
- Changed the titles of the module to fit inside the 32 character max of the payment_type field of the order table
- Added to the email footer to let them know they can print a form from account history.

This module is still only written for and tested on ms1.

The fax form has fairly basic formatting, feel free to play around with it yourself.

It works on my MS1 install....please test on later snapshots and report to the forum, also the form prints fine through IE6 on my hp laser...please test on other printers, browsers etc.

The purpose of the module is to provide a payment method for customers who despite SSL pages
and privacy policies still don't like submitting credit card details over the internet but
who wish to pay by credit card for their convenience or so that they don't have to wait for
cheque clearance etc before the goods ship.
It may also be of benefit in circumstances where they are buying bespoke or out of stock items
where there is a delay before shipment and you consider it unfair to actually process the credit
card at the time of order automatically.
Or maybe you only have a manual credit card processing system and not an online gateway.

I will release a further version shortly which will automatically create either (or both of) a printable fax sheet
page with all the order details completed, or send an email with all the details on which can then be printed and
faxed.
