CCCVV_ISSUE_START Selectable with card blacklist
This is an update to the CC_CVV+UK Switch contribution.
I have used a new title to remove confusion that it is not just for UK use as the collection of the Issue number and cvv can be turned on and off via admin.

Just fix a little problem in the blacklist.php file, in line 153 that fis a fatal error sending in the admin page.
The main change in this version is the addition of a pop up window for CVV help. This was obtained from another contribution and so all credit must go to jchasick for this feature.

- Added most of the updates that have been mentioned in forum (up to 15 Feb 2004) e.g. cvv field lengths, MasterCard ranges
As with version 1.6 not all changes are listed in the readme file, use a compare program to add all the required changes into your files. I have not had time to update this so perhaps someone else could update the readme file to polish the contribution off

Added encryption of the cc number in the database. Once encrypted it remains that way. The number is viewed only in admin/orders.php where it is decrypted 'on the fly' as the page is written to the client browser.
After a request this Update adds functionality to turn off/on start date collection via admin.
